Steps 1 through 6, enquiry through certificate issuance and token loading, can all happen the same day. Physical token delivery (step 7) then takes 1-3 days depending on your location.
Usually a connectivity issue, we simply reschedule. If it's an Aadhaar/PAN mismatch, that needs correcting with UIDAI or the Income Tax department first.
For Document Signer certificates, yes. Signing and Combo DSCs for portal filing are issued on a USB token as standard, since most portals require hardware-based storage.
